A Chronic Special Needs Plan (C-SNP) is a type of Medicare Advantage plan built for people living with a specific severe or disabling chronic condition. Instead of a one-size-fits-all plan, a C-SNP shapes its provider network, drug list, and care coordination around that condition.
Conditions that can qualify
Medicare defines the chronic conditions that a C-SNP can serve. Common examples include:
- Diabetes mellitus
- Chronic heart failure and certain cardiovascular disorders
- Chronic lung disorders such as COPD
- End-stage renal disease (ESRD) requiring dialysis
